Oxygen-ozone therapy · Segrate, Milan

Switch off inflammation,
stop degenerative processes.

We fight not only inflammation but also pain, to restore quality of life.

Chronic inflammation is the silent driver behind persistent pain and the degeneration of discs, cartilage and tendons. Oxygen-ozone therapy works at the root of this process, with a high safety profile and without the side effects typical of long-term medication.

The root of the problem

Chronic inflammation: when defence becomes disease

Inflammation is, at its origin, a defence mechanism: it isolates damage and initiates repair. The problem arises when it does not switch off. Low-grade chronic inflammation stays active for months or years, keeps tissues in a permanent state of alert and produces two consequences: persistent pain and the progressive degeneration of the joints.

At the molecular level, persistent inflammation is sustained by NF-κB, the main pro-inflammatory transcription factor, which stimulates the production of cytokines (TNF-α, IL-1β, IL-6) and free radicals. This oxidative environment damages cells, sensitises nerve endings and accelerates tissue wear.

Treating pain therefore means acting on this vicious circle — not merely masking the symptom — favouring effective and well-tolerated therapies that do not require long-term use of anti-inflammatory drugs, with their effects on the stomach, kidneys and cardiovascular system.

From symptom to mechanism

Inflammation and degenerative processes

How chronic inflammation turns a passing discomfort into a degenerative condition.

Degenerative musculoskeletal processes — osteoarthritis, disc degeneration, chronic tendinopathies — are not simply the result of age or mechanical wear. They are biologically active processes, in which inflammation plays a central role in maintaining and accelerating the damage.

Intervertebral disc degeneration

The intervertebral disc is a poorly vascularised structure, nourished by diffusion. When it cracks or bulges, material from the nucleus pulposus comes into contact with the immune system and triggers an intense inflammatory reaction. It is the inflammatory cytokines, even more than simple mechanical compression, that irritate the nerve root and generate the pain of sciatica and low back pain. Reducing local inflammation therefore means addressing the real cause of the pain.

Osteoarthritis and cartilage degeneration

In osteoarthritis of the knee, hip or shoulder, the cartilage thins progressively. The process is fuelled by enzymes (metalloproteinases) activated precisely by inflammatory cytokines: more inflammation means more degradation of the cartilage matrix, which in turn releases fragments that feed further inflammation — a self-sustaining cycle.

Chronic tendinopathies

In tendinopathies (shoulder, elbow, Achilles tendon) the tendon tissue loses its ordered structure and is replaced by disorganised, weaker tissue. Here too, the oxidative, inflammatory environment hinders the tendon's natural repair.

Oxidative stress and tissue ageing

The common denominator is the redox imbalance: an excess of free radicals compared with the cell's antioxidant defences. This imbalance is now recognised as one of the fundamental mechanisms of tissue ageing. Acting on it means not only reducing pain, but supporting the repair capacity of tissues over time.

The therapy

Oxygen-ozone therapy: how it works

A targeted response to inflammation and oxidative stress, with high tolerability.

Oxygen-ozone therapy uses a mixture of oxygen (O₂) and ozone (O₃) in controlled concentrations for therapeutic purposes. Medical ozone cannot be bottled: it is produced on the spot with dedicated equipment and dosed precisely by the physician for each condition and treatment area.

The mechanism is based on an apparently paradoxical principle, oxidative preconditioning: controlled amounts of ozone generate a mild, transient oxidative stimulus that "trains" the cell to activate its own defence systems. The Nrf2 pathway is activated, endogenous antioxidant enzymes increase (superoxide dismutase, catalase, glutathione peroxidase) and NF-κB is simultaneously curbed, with a consequent reduction of inflammatory cytokines.

In addition, it improves tissue oxygenation — particularly important in poorly vascularised structures such as the intervertebral disc and tendons, where low oxygen helps sustain the degenerative process.

Anti-inflammatory action

Modulates the mediators of inflammation, activates the Nrf2 pathway and reduces the expression of the pro-inflammatory cytokines responsible for persistent pain.

Antinociceptive action

Reduces the sensitisation of nerve endings and pain, with particularly strong evidence for low back pain caused by a herniated disc.

Antioxidant and anti-ageing action

Strengthens endogenous antioxidant defences, supporting the redox balance underlying the processes of tissue ageing and degeneration.

Systemic ozone therapy

Autohemotherapy: major and minor

Beyond local injections, ozone can act on the whole body through the blood, with a systemic antioxidant and immunomodulating effect.

MAH

Major autohemotherapy

A volume of venous blood (usually 100–200 ml) is drawn in a closed, sterile circuit, mixed with a precise amount of oxygen-ozone at a controlled concentration (typically 10–40 µg/mL) and reinfused. The usual cycle is 5–10 weekly sessions, repeatable 1–2 times a year. It stimulates antioxidant defences (Nrf2 pathway), modulates the immune response and improves tissue oxygenation and microcirculation.

mAH

Minor autohemotherapy

A small amount of blood (a few millilitres) is mixed with ozone and re-injected intramuscularly. It works mainly as an immunomodulating stimulus — a kind of biological "auto-vaccine" — useful in some allergic and immune conditions. It is a quick, outpatient procedure.

Vascular and microcirculation disordersPeripheral arterial disease, venous insufficiency, heavy legs.

Frequently asked questions

The most common questions

What is oxygen-ozone therapy?
It is a medical practice that uses a mixture of oxygen and ozone in controlled concentrations for therapeutic purposes. Medical ozone is produced on the spot with dedicated equipment and administered by the physician locally (injections) or systemically, according to defined protocols.
Is ozone therapy painful?
Injections are generally well tolerated. Fine needles are used and the reported sensation is mild pressure or a brief sting. Normal activities can usually be resumed after the session.
How many sessions are necessary?
It depends on the condition and the individual response. For musculoskeletal cases a cycle of 6-10 sessions is usually planned, weekly or twice weekly. The exact number is defined after the assessment visit.
Are there side effects or contraindications?
Injectable ozone therapy has a high safety profile: in clinical studies the incidence of side effects is very low and the events described are generally mild and transient. There are, however, some contraindications (including favism/G6PD deficiency, uncontrolled hyperthyroidism and pregnancy), which are always assessed at the visit.
Does ozone therapy work for a herniated disc?
It is one of the most studied applications. Systematic reviews and meta-analyses of randomised trials have documented a significant improvement in pain and disability in low back pain secondary to disc herniation, with a good safety profile.
Can I have ozone therapy if I take medication?
In most cases, yes. It is essential, however, to inform the physician of any current medication — in particular anticoagulants and antiplatelet drugs — so that the safest treatment can be selected.

About me

Dr. Domenico Bruno Pagano

Anaesthesiologist · Intensivist · Specialist in Pain Management and Oxygen-Ozone Therapy

An anaesthesiologist and intensivist, I focus on pain management and oxygen-ozone therapy. My approach stems from the belief that caring means looking after the person as a whole, not just treating an isolated symptom.

I obtained my specialisation in Anaesthesia, Intensive Care and Pain Medicine in Florence. My experience in anaesthesia and intensive care has taught me to see pain as a complex phenomenon with a precise biological basis. That is why, faced with persistent pain, the first goal is to understand which process — almost always inflammatory — is feeding it, and to act on that.

For each patient I build a tailored pathway that starts from an accurate clinical assessment and favours conservative, minimally invasive treatments, with the aim of reducing pain and restoring quality to everyday life.

I am a member of SIOOT — the Italian Scientific Society of Oxygen-Ozone Therapy and I follow its standardised clinical protocols.
